Purpose
Add the standard header to a facility SOP page.
Usage
Syntax

Parameters
Facility - description of the facility this SOP belongs to
Version - incrementing version number - matching the changelog as applicable.
Title - (Optional) override title syntax for misc. non-SOP documents.
